Job Description

Provides continuum of care for patients from admission to discharge.  Assesses social, emotional, and financial needs of the patient.  Assists in developing discharge plans with patients and their families.  This includes referrals to community resources and/or placement in an appropriate facility, following discharge.  

Education:   

• Masters degree from a recognized school of Social Work accredited by the Council on Social Work Education

Experience:    

• Minimum one year of job related experience preferred in an acute care setting
• Knowledge of Medicare/Medicaid regulations
• Knowledge of community resources.

Licensures/Certifications:  

• Current LMSW South Carolina License.
• BLS
