FTE: 1.0, Shift: Day, Schedule: 5/8's, May fill shifts starting at 7:00am or 8:30am

Position Summary

The Care Coordinator is responsible for coordinating healthcare services to manage the healthcare needs of recipients. This includes logistical support, advocacy and education to assist individuals in understanding the healthcare system and activities that help recipients gain access to needed physical and behavioral health services and manage their health conditions. The incumbent provides direct assistance in gaining access to services, coordination of care within and across systems, oversight of the entire case, and linkage to appropriate services. This position directly works with interdisciplinary team to coordinate patient care within a person-centered healthcare home

Responsibilities

  • Direct assessment of the eligible recipient to determine service needs

  • Development of an individualized integrated care plan (IICP); referral and related activities to help the recipient obtain needed services

  • Monitoring, follow-up, and evaluation

Requirements

  • Bachelor’s degree and two (2) or more years experience in social sciences, mental health or related field required; a Master’s degree is preferred

  • Washington State Agency Affiliated Counselor within 60 days of hire, unless one of these higher credentials are held: Counselor Agency Affiliated Registered, Counselor Agency Affiliated Certified, Mental Health Counselor Assoc (LMHCA)

  • Driver License/Abstract

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $20.88 - $30.05 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant’s years of experience align.
