Job Summary:

Locations: Methodist Community Health Clinic
Address: 208 S 26th Ave, Omaha, NE 68131

Schedule: Monday/Tuesday, 8am - 4:30pm, Wednesday - Friday, 7am - 3:30pm.

Provide professional nursing care to patients and families in accordance with individualized patient needs, nursing standards and the established plan of care. Perform assessment, planning, intervention, coordination and evaluation of care across a variety of inpatient, outpatient, procedural and clinic-based environments.

Responsibilities:

Essential Functions

Direct Patient Care – 50%

  • Assess patients and identify nursing care needs within the assigned setting.

  • Develop, implement and evaluate individualized plans of care.

  • Administer medications, treatments and therapeutic interventions in accordance with orders and policy.

  • Monitor patient status and respond to changes in condition in a timely manner.

  • Deliver nursing care in accordance with established standards, department workflows and assigned patient population needs.

Care Coordination and Team Collaboration – 20%

  • Coordinate care with physicians, advanced practice providers and interdisciplinary team members.

  • Communicate patient status, care needs and significant changes to the appropriate care team members.

  • Delegate tasks appropriately to support staff based on patient needs and staff competency.

  • Support patient throughout transitions of care and discharge planning activities.

  • Participate in handoff communication to ensure continuity and safety of care.

Documentation and Regulatory Compliance – 20%

  • Document patient assessments, nursing interventions, care provided and patient outcomes in the electronic health record.

  • Maintain compliance with hospital policies, departmental procedures and professional nursing standards.

  • Complete required competencies, mandatory education and department-specific training.

  • Follow infection prevention, patient safety and confidentiality requirements.

  • Support compliance with access and workflow requirements as applicable.

Patient and Family Education – 10%

  • Educate patients and families regarding the plan of care, medications, treatments and discharge instructions.

  • Reinforce health teaching and follow-up care needs using patient-centered communication.

  • Respond to patient and family questions and concerns in a timely and professional manner.

  • Promote a supportive and compassionate care environment.

  • Perform other duties as assigned in support of patient care and department operations.

Education

  • Graduate of an accredited school/college of nursing required

Experience

  • No Experience required

License/Certification

  • Current valid Registered Nurse (RN) License, valid compact multistate license, or a temporary permit while awaiting licensure.

  • Current American Heart Association or American Red Cross Basic Life Support (BLS) required within 3 months of hire; may be required at hire for some assignments.

  • Strong assessment, critical thinking, and clinical decision-making skills.

  • Ability to prioritize, multitask, and manage competing demands in a fast-paced environment.

  • Working knowledge of nursing practice, patient safety principles, and interdisciplinary care coordination.

  • Ability to function independently while exercising sound judgment.

  • Ability to support a broad range of patient care workflows depending on assigned department.
