Essential Functions:
Observation of client functioning and identify and report changes to supervisor.
Follow client’s written plan of care and report changes in calendar.
Complete appropriate records documenting daily activities.
Assist with household chores including but not limited to cooking, meal preparation, dusting, cleaning dishes, vacuuming, sweeping/mopping floors and laundry.
Assist clients with completing activities such as shopping and appointments outside the home; provide necessary receipts and documentation in the conduct of essential shopping/errands.
Assist clients with daily living activities and personal care.
Assist client with ambulation, positioning, toileting, and transfers.
Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest and fair manner.
Position Qualifications:
High school diploma (or equivalent) or one (1) year experience in a comparable human service capacity, or one (1) year experience in caring for a dependent child or adult family member, or demonstration of continued progress towards meeting educational requirements of a general education diploma required.
Background check clearance required.
Minimum of one (1) year related experience required.
Completion of twelve (12) months in the program required.
Previous, documented and supervised training within past two (2) years, equivalent to eight (8) hours of homecare aide training preferred.
Successful completion of RN, LPN, ME, physicians assistant or CNA training preferred.
Verified/documented current HCA completing LSSI designated Specialized training preferred.
Demonstrated ability to be consistent with attendance; arrive at work on time and work the allotted hours in client’s Plan of Care.
Demonstrated dependability/reliability in service to clients, including notifying client/supervisor with any schedule changes.
Demonstrate understanding of Home Care timekeeping system with very few clock in/out errors.
Demonstrated ability to communicate in English in a clear, comprehensible manner, both verbally and in writing.
Valid driver’s license, in good standing for the state of residency required.
Access to reliable transportation required.
Valid IL statutory minimum liability insurance coverage, bodily injury and property damage required.
