Territory: We are now expanding our high-quality care throughout Palm Beach County, hiring skilled PTs to join our team. This position covers our North Palm Beach Zone, including Jupiter and Palm Beach Gardens.

What You’ll Do:
Provide high-quality, one-on-one physical therapy to geriatric patients in their homes.
Complete Evaluations, Re-Assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge).
Create and implement effective plans of care (POC) to improve patient mobility and safety.
Coordinate directly with our clinical director and as needed, the patient's physician.
Compensation (1099 / PRN):
Rates start at $65 and up depending on experience and the visit type.

What You Need:
Active Florida Physical Therapist (PT) License.
Current CPR/BLS Certification.
Valid Driver’s License and reliable transportation.
Home Health experience preferred (Kinnser/WellSky and OASIS experience is a plus).
