Care Coordinator - can be RN, LPN or MSW

Nature & Scope of Position

The Care Coordinator is primarily responsible for coordinating care for an assigned panel of beneficiaries/clients to maximize the potential of the client and provide them with optimal healthcare services. The position is also responsible for assessing, planning, implementing and evaluating patient’s health care needs. An integral member of the health care team, coordinates patient care, communicates, and collaborates extensively with other members of the health care team, patient’s family and or legal guardian.
