Registered Nurse, Assertive Community Treatment (ACT) - Oakland County

The Assertive Community Treatment (ACT) Registered Nurse serves as a member of community-based mental health team that provides services for individuals experiencing serious mental illness. The ACT Registered Nurse provides integrated behavioral and physical healthcare including medication management, the assessment and coordination of physical/medical care, and the provision of direct services to the beneficiary in the community. The mission of ACT is to help people become independent and integrate into the community as they experience recovery through a team-based service model.
