Coordinating these services to clients in CCBHC program in a client-centered and culturally responsive way. Provides advocacy, referral linkage, and rehab services within a multidisciplinary team to adult clients within the Counseling For Children & Adults program. The clients served in this program have diagnoses of Anxiety, Depression, and PTSD among others. Family & Children's Services utilizes the Certified Community Behavioral Health Clinic (CCBHC) model of care. As a member of a multi-disciplinary care team, you will successfully support clients by providing whole person care with this transformative model.
