Case Manager, COPES Team

COPES elite team now hiring! $58K/year +$3K Premium, generous benefits and PTO! Must have degree in social sciences +2 yrs behavioral health exp. Work with an extraordinary team providing follow up services to individuals that have been assessed by a crisis team. Provide client advocacy and assist individuals with accessing all levels of community resources needed to recover. Provide crisis services which divert individuals experiencing mental health crisis from inpatient hospitalization, jail, homelessness or emergency departments. Family & Children's Services utilizes the Certified Community Behavioral Health Clinic (CCBHC) model of care. As a member of a multi-disciplinary care team you will successfully support clients by providing whole person care with this transformative model.
