Job Description

Location – Fully remote role, office days required on occasion for training or meetings in Sydney Flexible hours and days – 3 - 4 days per week between Monday and Friday   Pay rate – $56.73 per hour + superannuation   Up to $18,550 tax free with Not For Profit Salary Packaging    Aged Care Assessment Services (ACAS) positions offer flexibility and autonomy and are based across both metropolitan and regional locations, with most roles involving some local travel within each region from time to time. We have an exciting opportunity for a Clinical Triage Delegate to join the team on a 12 month fixed term contract covering parental leave (potential to extend or become permanent).  What will your role look like?  Your role will assist older people in navigating the assessment process for aged care services by conducting thorough triage evaluations to identify the appropriate assessment pathway and prioritize their needs.  Day to day this includes:   Establishing rapport to create a safe and comfortable environment for the client to express their concerns and to confirm screening information utilising the Integrated Assessment Tool (IAT) Tool   Determining eligibility and assessment priority and pathway   Conducting risk assessments based on client vulnerability    Gathering information relevant to client circumstances and confirming the information within the referral   Determining eligibility and assessment priority and pathway   Conducting risk assessments based on client vulnerability    Maintaining currency of knowledge relating to the Aged Care Act and relevant Aged Care frameworks and standards        To be successful in this role you’ll need:   At least one year’s experience as a Triage or Clinical Delegate or Clinical Aged Care Assessor (essential) To be bilingual – preferred languages are Mandarin, Cantonese, Vietnamese and Arabic (desirable) Excellent Communication and Customer Service skills to both internal and external stakeholders   Excellent planning, organising and problem-solving skills     Effective negotiation, advocacy and conflict resolution skills     Tertiary qualifications – in a health-related discipline directly related to health, aged care or related specialist area.
