Family Coach- Rapid Rehousing

This position provides home and community based one-on-one services to clients that have recently moved from homeless to housing. This position provides a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy to meet an individual’s and family’s comprehensive needs through communication and available resources.
