Behavioral Health Clinician - Home-Based

Provides individual, group, conjoint or family psychotherapy and support services to children with severe emotional disturbance and their family. Conducts clinical assessments, develops diagnoses, and participates in developing person/family centered treatment plans with client families. Monitors client progress and coordinates appropriate behavioral health and other needed services. Responsible for meeting all MDCH requirements for Home-based hours (minimum 4 hours per family per month). Maintains an electronic and paper chart which includes required documentation of services provided, progress notes, treatment plans, periodic reviews and client rights documents.
