Schodack Internal Medicine and Pediatrics, part of Community Care Physicians (CCP), is a well-established primary care practice providing comprehensive care for patients of all ages in a welcoming, team-based environment. Our collaborative care team includes a board-certified pediatrician, board certified Med/Peds physicians, nurse practitioners, and physician assistants dedicated to deliver coordinated, high-quality care

Position Summary
We are seeking a full-time Physician committed to compassionate, evidence-based primary care. The ideal candidate values long-term patient relationships, quality outcomes, and collaborative team-based practice.
Key Responsibilities
- Provide high-quality clinical care to patients of all ages in internal medicine, family medicine, or Med-Peds practice.
- Perform comprehensive history and physical examinations, develop assessment and care plans, and coordinate treatment with care team members.
- Manage acute and chronic conditions, including preventive care and health maintenance.
- Provide same-day sick visits, wellness visits, chronic disease management, and age-appropriate screenings.
- Collaborate with nurse practitioners, physician assistants, nursing staff, and administrative personnel to ensure timely, patient-centered care.
- Utilize electronic health records and digital tools to support continuity and access to care.
- Participate in quality improvement initiatives and practice performance activities.
- Educate and counsel patients and families to promote health, wellness, and disease prevention.
Qualifications & Requirements
- Ideal Candidate: BC/BE Internal Medicine and Pediatrics (Med-Peds) Physician or BC/BE Family Medicine Physician
- MD or DO degree from an accredited medical school.
- Valid and unrestricted New York State medical license (or eligibility).
- Strong clinical, diagnostic, and communication skills.
