Position Overview
Finger Lakes Health is seeking a detail-oriented and experienced MDS Coordinator (RN) to join our Living Centers in Geneva, NY. In this key role, you will lead all MDS assessment, care planning, and reimbursement activities—ensuring compliance with regulatory standards while supporting high-quality resident care.

Key Responsibilities
- Coordinate and oversee all MDS assessments in accordance with federal and New York State regulations
- Develop, manage, and communicate MDS schedules with the interdisciplinary care team
- Ensure timely and accurate completion and submission of MDS, CAAs, and care plans
- Monitor and interpret Quality Measures to support clinical outcomes and compliance
- Lead and support reimbursement processes, including decision-making and denial management
- Serve as a liaison between Nursing, Therapy, and Resident Accounts
- Provide documentation oversight and ensure compliance with reimbursement guidelines
- Identify documentation gaps and educate staff through training and in-services
- Support Medicaid reimbursement through accurate MDS census data submission
Qualifications
Required:
- Current New York State RN license
- Graduate of an accredited Registered Professional Nursing program
- Experience with MDS, care planning, and reimbursement documentation
- Knowledge of NYSDOH regulations and survey process
- Strong organizational skills with the ability to manage multiple priorities and deadlines
- Demonstrated professionalism, discretion, and teamwork
- Computer proficiency
Preferred:
- BSN
- Certification in MDS and PRI
- Experience with MDS/care planning software and Microsoft Office
- BLS certification (or willingness to obtain upon hire)
