The Youth Care worker assists with the day-to-day operations of the Youth Services Program. They carry out responsibilities in the following functional areas: role modeling, goal setting, independence, recreational activities, member rights, personal cares, daily living activities, and regulatory compliance.
ESSENTIAL FUNCTIONS AND DESIRED OUTCOMES:
- Ensuring a secure, safe, and nurturing environment for youth, including monitoring behaviors and managing crises.
- Act as a positive role model, guiding, and mentoring youth to develop life skills, self-esteem, and positive behaviors.
- Lead recreational activities, group sessions, and educational workshops to promote positive growth.
- Provide comprehensive training to all Members through individual and group activities.
- Implement behavior plans and aim to foster positive behavioral changes.
- Escort youth to appointments or school.
- Teaches and guides youth in achieving hopes, dreams, goals and objectives through self-direction, decision-making, and goal setting.
- Provides personal cares as directed by individual needs in a member specific manner.
- Assists and monitors all activities of daily living as indicated in support plans and as desired by the Guardian/Care Team to include, but not limited to: personal hygiene, toileting, and independent living skills.
- Follows safety and regulatory procedures in all areas of food preparation, serving of meals, clean up and other related activities.
- Prepares and serves meals by following general nutritional guidelines, menus, individual dietary restrictions, needs and preferences to ensure member health.
- Assists, monitors, and documents medication administration to ensure member health and compliance with Department of Human Services, Care Management Organizations, and Clarity Care policies and procedures.
- Follows all documentation and reporting policies and procedures for any medication and/or safety issues, and significant events.
- Coaches, monitors, reports and documents youth’s verbal and physical behaviors to ensure the safety and health of youth and others.
- Maintains a safe, sanitary and welcoming environment by performing basic cleaning of the program, laundry, etc.
- Responds promptly and competently in emergency response situations.
- Assists, escorts, and provides physical and other types of assistance needed by youth during social and recreational activities.
- Provides opportunities sensitive to the needs, preferences, and choices of the youth while promoting community recreational participation.
- Contributes to Clarity Care team effort by accomplishing related results and tasks as needed or assigned.
- Act as a mandated reporter to report any potential abuse and/or neglect.
EDUCATION/QUALIFICATIONS:
- Must possess a high school diploma/equivalent and/or related work/life experience.
- Prior experience working with youth, particularly those with behavioral or mental health challenges, strongly preferred.
- Must be at least 21 years of age.
- Must possess a valid WI driver’s license, and ability to meet agency driving standards. Must provide proof of auto insurance.
- Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence.
