Outreach Caseworker

The Old Colony Y Intensive Home based Therapeutic Care Program is seeking energetic and flexible outreach staff who have a passion to make a difference for children and families in Southeastern MASS communities. The Outreach Caseworker will be part of the Intensive Home Based team working with an assigned caseload of youth and families referred by the Department of Mental Health. The Outreach Caseworker will be comfortable working both independently and collaborating with the team in supporting youth experiencing mental health needs that impact their home and community relationships. This role involves various treatment oriented services, family engagement, advocacy, life skills coaching, and connecting youth and families to local community resources. This role also involves crisis interventions to support families, participation in family team meetings, care coordination, treatment documentation, and group staff development meetings.
