Job Description
The Care Team Hospice is seeking a compassionate, organized, and detail-oriented Intake Specialist to join our team. This is a critical role for someone who thrives in a fast-paced healthcare environment and enjoys being the connection point between referral sources, patients, families, and clinical teams.
As an Intake Specialist, you will play a key role in ensuring a smooth and timely admission process for patients entering our care. You'll coordinate referrals, verify insurance information, communicate with referral partners and clinical staff, and help ensure patients and families receive the support they need during an important time in their healthcare journey.
We are looking for someone who is highly organized, customer-service focused, and able to manage multiple priorities while maintaining accuracy and professionalism. The ideal candidate will have experience in healthcare intake, hospice, home health, or a related clinical setting and be comfortable working with electronic medical record systems and referral platforms.

Key Responsibilities:
• Process and coordinate new patient referrals, ensuring accurate and timely intake documentation and follow-up
• Communicate with referral sources, patients, families, and clinical teams to facilitate admissions and ongoing care coordination
• Verify insurance benefits, authorizations, and coverage information while maintaining accurate patient records in the EMR
• Schedule admissions, nursing visits, and other clinical services while monitoring referral and intake workflows to meet service timelines
• Provide exceptional customer service and administrative support by answering phones, managing inquiries, and ensuring seamless communication across departments
Qualifications:
• High school diploma or equivalent required; additional education preferred
• Two or more years of healthcare intake, hospice, home health, or related healthcare experience preferred
• Knowledge of medical terminology and healthcare operations preferred
• Experience with EMR systems and referral platforms such as Homecare Homebase, CarePort, NaviHealth, Allscripts, or similar systems preferred
• Strong communication, organizational, and multitasking skills
