Patient Navigator - Dental

The Patient Navigator is a member of the clinic interdisciplinary team responsible for facilitating improved health care access and quality for BRH patients through advocacy and care coordination to address social determinants of health (SDH). Responsibilities include: - Screens patients for the eligibility of and assists with completion of needed applications and paperwork for: - sliding fee scale/discount fee program - subsidized health insurance programs - charity care programs, including local hospital applications - medication assistance programs - addressing legal issues - other programs that assist with social determinants of health (i.e. food insecurity, housing insecurity, transportation...
