Hospice On-Call Licensed Practical Nurse (LPN)
After-hours support. Mostly remote. Critical impact.
The Care Team is seeking a Licensed Practical Nurse (LPN) to join our hospice team in a 7-on/7-off on-call role. In this position, you will provide skilled nursing support to patients in their homes or facility settings under the direction of a Registered Nurse, in accordance with the Pennsylvania scope of practice. You will play a key role in after-hours care, supporting patient comfort, responding to urgent needs, and ensuring timely communication with the RN and interdisciplinary team.
Schedule & Coverage
• 7 on / 7 off on-call schedule, working remotely, and dispatching in the field as needed.
• Weekday hours: 4:30 PM to 8:00 AM
• Weekend coverage: Friday 4:30 PM through Monday 8:00 AM
• Assigned coverage area based on branch needs and patient census

Key Responsibilities
• Provide nursing care as delegated by the RN in accordance with the plan of care
• Respond to after-hours patient needs, including symptom support and urgent visits as directed
• Observe patient condition and promptly report changes to the RN
• Support comfort-focused care and reinforce education provided by the RN
• Document care and patient interactions accurately in Homecare Homebase (HCHB)
• Collaborate with the interdisciplinary team to ensure continuity of care
• Active LPN license
• Hospice experience preferred
• Experience using HCHB (Homecare Homebase) preferred
Field Role Requirements
• Active driver’s license
• Ability to pass a criminal background check
• Ability to pass a Motor Vehicle Record (MVR) check
• Current TB testing in accordance with agency policy
• Current CPR certification as required for the role
