Hospice Clinical Team Lead (RN)
Step into leadership without leaving the bedside.

 

We are seeking a Clinical Team Lead (RN) for a dual role that supports and partners with the Branch Administrator and Clinical Manager while also carrying a small patient caseload in the field. This position is part of the leadership team and plays a key role in ensuring quality patient care, supporting clinicians, and maintaining efficient clinical operations. 

Schedule, Hours & Territory 
• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM 
• Assigned territory based on patient census and branch needs 
• You will carry a small caseload based on branch needs while also spending dedicated time in our Bethlehem, PA office supporting the clinical team.

Key Responsibilities
• Provide direct patient care with a small caseload
• Support daily clinical operations and staff needs
• Review documentation, orders, and care plans for accuracy
• Assist with IDT meetings and care coordination
• Help train and support clinical staff
• Communicate with physicians, patients, and families
• Assist with referrals, scheduling, and workflow
• Ensure compliance with policies and regulatory standards

• Active RN license required
• Hospice or home health experience preferred
• HCHB experience preferred
• Strong clinical, organizational, and communication skills
• Ability to balance leadership support with patient care

Field Role Requirements 

• Active RN license in the state of Pennsylvania
• Active driver’s license 
• Ability to pass a criminal background check 
• Ability to pass a Motor Vehicle Record (MVR) check 
• Current TB testing in accordance with agency policy 
• Current CPR certification as required for the role
