Job Description

Community Healthcare of Texas has provided hospice and palliative care services since 1996.  Our mission is to provide compassionate care for individuals living with serious illnesses while supporting their loved ones who care for them.

Community Healthcare of Texas is currently recruiting for a Clinical Nurse Liaison - RN. 


POSITION SUMMARY

Serve as the hospice expert to professionally and seamlessly coordinate services and bridge the transition for the patient from the hospital to hospice services; develop long-term relationships at each facility to reinforce CHOT/Providence Hospice as the provider of choice. 

ESSENTIAL FUNCTIONS

  • Respond to referral sources promptly; evaluate patient appropriateness for hospice; meet with families/patients to discuss goals and explain services of hospice
  • Complete required paperwork with patient/families; facilitate patient discharge by working with hospital staff, DME, medications, hospice physicians, and hospital staff
  • Educate hospital staff and follow up on pending patients; round daily to be available to staff and patients for questions and to build relationships
  • Communicate and provide daily feedback to admission staff on referrals
  • Complete thorough documentation of hospice eligibility in the required timeframe
  • Through customer service and leveraging relationships, become the “hospice of choice,” thereby increasing hospital referrals
  • Attend and participate in routine staff meetings in person or via media provided as directed
  • Participate in and/or provide in-service training, attend various hospital activities, and perform other duties as required or assigned
  • When a patient is admitted to GIP, complete an initial, comprehensive assessment of patient and family to determine hospice needs. Provide a complete physical assessment and history of current and previous illness(es)
  • Assess and evaluate patient's status by:
    • Writing and initiating a plan of care
    • Regularly re-evaluating patient and family/caregiver needs
    • Participating in revising her plan of care as necessary
  • Initiate the plan of care and make necessary revisions as patient status needs change; include the patient and the family in the planning process
  • Complete, maintain and submit accurate and relevant clinical notes regarding patient’s condition and care given. Record pain/symptom management changes/outcomes as appropriate
  • Communicate with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtain/receives physicians’ orders as required
  • Communicate with hospice staff to coordinate the care plan
  • Work in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family

POSITION QUALIFICATIONS

  • Registered RN in the State of Texas required  
  • Nursing experience or medical training required (ability to read charts, know medications, etc.) 
  • Marketing experience required; 1-3 years of experience in healthcare marketing preferred 
  • Hospice experience strongly preferred 
  • Proficient computer skills are required
