Aftercare Coordinator

Banyan Treatment Centers is seeking a detail-oriented Aftercare Coordinator to support patients as they transition from treatment to the next level of care. This role coordinates discharge planning, communicates with families and referral sources, and ensures each patient leaves with a clear, well-organized aftercare plan. The Aftercare Coordinator collaborates closely with clinical and operational teams while maintaining accurate documentation and compliance with regulatory and confidentiality standards to support continuity of care and long-term recovery.
