Join a team that prioritizes growth, open communication, and the tools you need to make a meaningful impact in the lives of others.Location: North Inland and Central Job SummaryWe’re looking for a skilled and compassionate LPN/LVN to provide in‑home nursing care under the supervision of a Registered Nurse (RN). You’ll support patients with daily care needs, monitor their condition, and help ensure safe, high‑quality home health services.ResponsibilitiesProvide nursing care according to the RN‑developed Plan of Care.Monitor and document changes in patient condition; report to RN/physician as needed.Assist with activities of daily living to promote patient independence.Educate patients and caregivers within LPN/LVN scope of practice.Maintain accurate EMR documentation.Participate in case conferences and coordinate care with the home health team.Follow infection‑control, HIPAA, and company policies.Current LPN/LVN license in the state of practice.Graduate of an accredited practical/vocational nursing program.Minimum 1 year acute care experience preferred.Current CPR/BLS certification.Valid driver’s license, reliable insured vehicle.Strong communication, clinical judgment, and ability to work independently.Qualifications •Graduate of a physical therapy school approved by the Council of Medical Education of the AMA in Association or its equivalent.•Licensed or registered to practice as a PT in the state of practice.•Preferably at least two (2) years of experience as a PT.•Possesses and maintains good physical stamina and mental health.
