Position Summary Assistant Director of Subacute Services to help lead and grow our short-term rehabilitation and post-acute care program.

This operational leadership role serves as a key liaison between clinical, admissions, therapy, case management, social work, and nursing teams to ensure exceptional outcomes for residents receiving short-term rehabilitation services. The ideal candidate is a highly organized leader who understands the unique needs of post-acute care and rehabilitation. Candidates with experience in SNF, rehabilitation operations, healthcare administration, reimbursement are encouraged to apply.
