JOB DESCRIPTION: MDS/CARE PLAN COORDINATOR FACILITY: DEPARTMENT NAME: Nursing UNIVERSAL PRECAUTIONS CATEGORY: 1, 2 & 3 IMMEDIATE SUPERVISOR: DIRECTOR OF NURSING ADMINISTRATION EDUCATIONAL REQUIREMENTS: Must be licensed as an R.N. or L.P.N. in this state. Must possess a current, unencumbered license to practice as in this state. Must be able to write and speak the English language in an understandable manner. Must possess the ability to make independent decision when circumstances warrant such action. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies and the general public. Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care. Must possess leadership and supervisory ability and the willingness to work harmoniously with professional and non-professional personnel. Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, and procedures, etc., that are necessary for providing quality care. Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents, family and/or staff member. Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices. EXPERIENCE: Must have a minimum of one year experience in a supervisory capacity in a hospital, a skilled Nursing Facility or other related healthcare program. Must possess a working knowledge of Long Term care operational standards as set forth in Federal Register, Conditions of Participation and State regulations. OVERALL PURPOSE: The primary purpose of your job position is to complete the RAI process, for each resident of the facility, as required by State and Federal regulations. This includes transmission and verification of receipt by the State. Also, maintain compliance with our own policies and procedures. Ensure that the highest degree of quality care is maintained at all times. As the MDS/Care plan Coordinator, you are delegated the administrative authority, responsibility, accountability necessary to carry out your assigned duties. JOB FUNCTIONS: Every effort has been made to make your job description as complete as possible. However, it in no way states or implies that these are the only duties you are required to perform. JOB DUTIES: Interpret the facility's policies and procedures to employees, residents, family members, visitors, government agencies, etc. as necessary. Complete the RAI process timely, for each resident in the facility. Conduct the Care plan meeting and formalize the written care plan utilizing the information obtained at the meeting. Include the resident and family. Ensure that personnel involved in providing care to the resident is aware of the care plan and that the care plans are used in administering daily care to the resident. Develop and maintain a good working rapport with inter-department personnel. Attend staff meetings. Participate in the ongoing quality assurance program for the nursing department. Present the QI information from the State monthly at the QA meeting. Make necessary written and oral reports to the Director of Nursing Administration as required. Review charts to ensure that they are informative and descriptive of the nursing care provided and that they reflect the residents' response to nursing care as it relates to the care plan. Report problem areas to the Director of Nursing Administration. Develop and implement corrective action. Serve on various committees of the facility: Care Plan, Quality Assurance, Utilization review, etc. Participate in Nursing "on call" rotation.
