Back to jobsPartner GPs in social prescribing by connecting patients to community resources to improve their overall health and wellbeing. This includes referral of patients to community and social partners for various care services such as community groups, exercise programmes, support groups as well as day care or home care services;
Support GPs in better integration and coordination of patient care with various community / social partners;
Provide technical support to elderly or less tech-savvy patients in using HSG-related mobile applications such as HealthHub, Healthy 365 Apps.
Work closely with the PCN nurse to identify patients who require nurse counselling, spirometry and other relevant services;
Support patients in adhering to their care plans by providing simple lifestyle and behavioral modification advice.
