Registered Nurse - Hospice of Cincinnati - Case Manager - Optional

This position provides care for hospice patients in homes and long-term care settings on the Westside of Cincinnati, collaborating with an interdisciplinary team of physicians, CNPs, nurses, STNAs, Social Workers, Chaplains, Holistic providers and Volunteers. This is a Fulltime position (1.0 FTE), working 8-hour shifts. This team sees patients Monday - Friday, 8am-4:30pm. Candidate must have reliable transportation with insurance.
