Summary:
For this position, the selected candidate will be working with and for the person supported who is self-directing. The Arc of Livingston Wyoming will serve as the Fiscal Intermediary and will act as the employer of record. In this role, you will support a person with an intellectual and/or other developmental disability. The person helps create a plan that outlines the type of support you will be expected to provide.
Essential Functions:
For Community Habilitation:
- Work 1:1 with the person to achieve the goals that are outlined in their individualized plan(s), which may include things such as cooking, cleaning, shopping, socialization, recreation, etc.
- Support the person in community engagement with a focus on building meaningful relationships and building social capital
For Respite:
- Provide care and supervision to the person primarily in their home setting.
Non-essential Functions:
- Must attend all required trainings
- Must adhere to agency policy and procedures
Reporting Responsibilities:
Reports to: The person supported (and their representative) provide supervision and oversight, while the Fiscal Intermediary serves as the employer of record.
Supervisory Responsibilities: None exercised
Knowledge, Skills, and Abilities:
- Must be motivated and willing to accept direction from the person supported (and their representative
- Must be able to read and understand a plan of serice and be able to support a person in the areas identified.
- As required, must be able to document supports provided to the person supported.
Requirements:
- Ability to lift/transfer 25 pounds.
- Must have stamina needed to provide functions outlined in the person’s plan.
- Must be able to walk up and down stairs.
Working Conditions:
- Would consist of working within the home of the person supported, as well as within the community.
Minimum Qualifications:
- A valid NYS Driver’s License (unless other arrangement is discussed and agreed upon in advance of hiring)
- Reliable transportation with current inspection (whenever transporting person supported)
- Must have adequate smart device (smart phone, tablet, laptop, etc) with internet access to document attendance and services
- Must be able to read, write and speak English
- Must possess a high school diploma or equivalent.
