LCSW Care Manager Psych - Evenings Full Time

LCSW Care Manager within our Psychiatric Department at Providence Little Company of Mary Medical Center in San Pedro, CA. This position is Full-Time and will work 8-hour Evening shifts (3pm - 11:30pm) to include weekend rotations.

Care Management within the Department of Psychiatry includes a collaborative practice with patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. Social workers facilitate communication and care coordination along a continuum through effective transitional care management.
