Position Summary: Reports to Program Director. Responsible for general health of all youth and monitoring provision of medical and dental care by community based providers, medication management, and health and wellness education to residents and families.
Essential Accountabilities:
ADMINISTRATION
1. Oversight of monthly medication log and resident's medical charts, i.e.: ensuring medication administration is documented accurately; medical chart maintenance is up-to-date, etc.
2. Arranges for staff training on specialized issues such as Medication Handling, HIV and Hepatitis, CPR and First Aid.
3. Oversees staff training of medication dispensation by providing yearly training for all staff and training for
newly hired staff.
4. Reports medication errors to Program Administration.
5. Submission of requires documentation in a timely manner in accordance with state and agency regulations.
OPERATIONAL
6. Ensures and adhere to OMH regulated 3: 1 daytime/evening and 4: 1 overnight youth to staff ratios at all times.
7. On call 24 / 7 for all health and medical issues of residents.
8. Responsible for addressing all day-to-day medical issues with residents.
9. Responsible for monitoring and following up with medication needs of residents.
10. Attendance of all monthly Psychiatric appointments and follow-up contact with Psychiatrist as needed.
11. Responsible for following up with medical / mental health providers regarding client medication issues.
12. Maintenance, review, and administration of all medications. Daily I weekly medication inventories, logging in of new medications, informing all staff of medication changes, and monitoring for side effects.
13. Conduct health and wellness groups with residents weekly. Monthly Health Services (HS) and Medication
Management Training (MMT) Medicaid Restorative Notes.
14. Primary contact person for hospital and other medical providers regarding medical issues.
15. Emergency coverage as needed (i.e.: staff shortage on shift, crisis CPEP visits after scheduled hours).
16. Ensure residents receive annual medical and biannual dental examinations.
17. Participate in monthly supervision with Program Director.
18. Participate in monthly clinical staff meetings.
19. Provides input during shift change meetings.
20. Provide direct care as needed. All residents must be in eyesight vision of staff at all times.
21. Provide hand-off communication with other staff members any time a resident enters or leaves an area of eyesight vision.
22. Transportation of clients as needed, sometimes use of personal vehicle as needed.
OTHER RESPONSIBILITIES
23. Responsible for tracking mileage with personal vehicle for company use and submitting mileage logs on last day of the month.
24. Swiping in at the beginning of shift and swiping out at end of the shift for all shifts worked including trainings at other Wellife Network sites. Submission of timesheets on Friday of the pay period ending.
25. Responsible for maintaining (or having access to) a personal vehicle throughout the course of the work day.
26. Submission of car insurance policy before the expiration date to supervisor.
27. Attendance and participation at the Family Bonds Retreat Weekend.
28. Other duties as assigned.

The Program Nurse will be required to have RN licensure with a minimum of 2 years adolescent experience (preferably in the psychiatric field). The Program nurse must also have strong communication and people skills, as well as knowledge of and experience working with adolescent population. Must have a valid New York State driver's license, and access to a vehicle.
