Social Worker, AAMC Community Health, Full Time, First Shift

This position is responsible for facilitating charity care access, navigating healthcare financing resources, and supporting underserved and uninsured populations in obtaining necessary medical services. A core focus of this role is addressing the complex needs of the aging population through coordination of long-term care placement, community-based long-term care services, and ancillary support programs. The Social Worker will function as a key liaison for the Medicare Program of All-inclusive Care for the Elderly (PACE), coordinating interdisciplinary services that support independence, dignity, and quality of life for eligible older adults in the Lakelands region.
