Service Coordinator - Residential Rehabilitation

[Shift: Full time, day shift] This is a direct service position requiring knowledge of mental health, community programming and entitlement programs. Primary responsibilities include the provision and/or coordination of assessing and monitoring client needs, developing service plans, linking clients to services and resources in the community, and advocating for clients as necessary.
