
Palliative Care Physician - Per Diem

Overview
The Palliative Care Physician will provide Palliative Medicine consultative services as a member of the CHS Symptom Management & Supportive Care Service team at assigned CHS healthcare facilities where the program provides services.
This position requires local travel up to 50% throughout the Long Island area to other CHSLI facilities. Position will be onsite at St. Joseph's Hospital in Bethpage when not traveling.

Responsibilities:
- Provide consultation services in palliative medicine as part of the Symptom Management & Supportive Care Service Interdisciplinary Team.
- Provide medical evaluation and treatment to patients on the service in both the inpatient and outpatient setting as assigned.
- Facilitate clarification of patient and family goals of care, advance care planning, and completion of advance directives as appropriate.
- Consult with the attending physician and the Interdisciplinary Team to establish a written plan of care and appropriate follow-up at intervals specified in the plan.
- Ensure that patients' physical and psychosocial symptoms are managed according to service and specialty standards of care.
- Conduct patient & family meetings to assess knowledge of disease status, establish goals of care, and discuss treatment options in concert with members of the Palliative Care Interdisciplinary Team & other providers, as applicable.
- Participation in on-call coverage of the Symptom Management Service to assure 24 hour, 7 days a week coverage.
- Participation in hospital programs & committee meetings as a representative of the Symptom Management Service as assigned.
Qualifications:
- Current license to practice medicine in New York State.
- Current Controlled Substance Registration Certificate and Infection Control certificate.
- Graduation from an accredited school of medicine or osteopathy or ECFMG Certification.
- Board certification in a primary medical specialty, internal medicine or family practice preferred.
- Subspecialty certification in Hospice and Palliative Medicine preferred; eligibility for Board. Certification in Hospice and Palliative Medicine or Hospice Medical Director Certification required.
- At least two years of postgraduate direct patient care in the absence of fellowship training in Hospice & Palliative Medicine.
