Position Highlights:

Major Responsibilities:

  • Endocrinology clinic-embedded pharmacist responsible for medication therapy for various chronic conditions including, but not limited to diabetes, thyroid conditions, osteoporosis, dyslipidemia, hypertension.
  • Maintains updated collaborative practice agreement, as applicable
  • Performs point-of-care tests according to CLIA guidelines and hospital regulations as well as ordering laboratory tests as clinically appropriate to monitor effectiveness of therapy (HbA1c and lipid panels).
  • Assesses patient needs and provides tools to support the patient in their self-care.
  • Designs, recommends, monitors, and evaluates patient specific pharmacotherapeutic regimens based on relevant biomedical literature. Implements recommendations effectively and utilizes policies and procedures and documents in the appropriate format. 
  • Uses effective patient education techniques to provide counseling to patients and caregivers, including information on medication therapy, adverse effects, compliance, appropriate use, handling, and medication administration.
  • Documents clinical activities in patients’ electronic health record to provide information needed to meet standards, regulations, laws and/or other administrative requirements
  • Updates knowledge and skills of self and other health care professionals to maintain, improve and document competency.
  • Participates in planning by establishing personal goals to support quality improvement efforts and contribute to the achievement of departmental objectives.
  • Practices in compliance with organization’s policies and procedures, as well as local, state, and federal regulations.
  • Serves as preceptor for pharmacy students, residents, and healthcare learners as assigned.
  • Mentors, orients, trains, and assists with the performance evaluation of pharmacy teammates, residents, and students as assigned.

Licensure, Registration, and/or Certification Required:

  • Bachelor’s or Doctorate degree in pharmacy from an ACPE-accredited school of pharmacy required, or an FPGEC certificate from NABP.
  • Active Licensure with the North Carolina Board of Pharmacy is required.
  • Clinical Pharmacist Practitioner (CPP) application for licensure will be initiated following employment. 
  • Board certification or advanced credentialing (e.g. BCACP, BCPS, CDCES) is preferred.

Experience Required:

  • Graduate of an accredited School of Pharmacy and completion of an ASHP-accredited PGY1 residency or equivalent experience is required.
  • PGY-2 residency in Ambulatory Care preferred.


Knowledge, Skills & Abilities Required:

  • Ability to communicate on a professional level with physicians, nurses and other professional staff when discussing medications and medication-related problems.
  • Excellent communication and organization skills.
  • Excellent presentation, facilitation, and training development skills.
  • Demonstrates decision making, team building, patient service, and conflict resolution skills.


Physical Requirements and Working Conditions:

  • Takes necessary precautions to avoid inadvertent needle sticks, minor cuts or bruises.
  • Exposed to normal office or clinic environment.
  • Must be able to perform manipulative skills such as writing, typing, and data entry into the computer.
  • Must have functional speech, vision, and hearing.
  • Exposed to various conditions throughout the workday such as walking on hard surfaces, climbing stairs and changes in temperature and humidity.
  • Operates all equipment necessary to perform the job.
