
Aftercare Worker (Hard to Place)
Job Description
Position Overview:
The Aftercare Worker provides supportive services to youth transitioning out of foster care into independent living. This role focuses on helping young adults develop life skills, access resources, maintain stability, and successfully navigate adulthood. The Aftercare Worker collaborates with youth, families, community providers, and internal teams to support positive outcomes in housing, education, employment, and overall well-being.
Position Qualifications:
Bachelor’s degree in social work, Psychology, Human Services, or related field (preferred). Associate degree with 4 years’ relevant experience may be considered. Experience working with foster care youth, adolescents, or young adults preferred. Knowledge of independent living skills and transition planning. Strong engagement and rapport-building skills with youth. Ability to work independently in community settings. Strong organizational and documentation skills.
