
PACE REHABILITATION AIDE (TEMP/CONTRACT)
Job Description
The Rehabilitation Aide provides direct and indirect support to rehabilitation staff within the PACE interdisciplinary care team. Under the supervision of licensed physical and/or occupational therapists, the Rehabilitation Aide assists in the delivery of therapeutic services to frail elderly participants, ensuring a safe, supportive, and culturally responsive care environment.
The PACE model is designed to enable seniors to remain living independently in the community by providing comprehensive medical, social, and rehabilitative services
ESSENTIAL JOB FUNCTIONS:
- Assist Physical Therapists and Occupational Therapists in implementing individualized treatment plans, including therapeutic exercises, ambulation, transfers, and activities of daily living (ADLs)
- Prepare treatment areas, maintain rehabilitation equipment, and ensure a clean and safe therapy environment
- Escort participants to and from therapy sessions and assist with mobility and positioning as needed
- Observe participant performance and report changes in condition or response to therapy staff
- Support group exercise programs and wellness activities as directed
- Maintain inventory of therapy supplies and assist with ordering items as directed
- Perform basic clerical duties, including documentation support and scheduling assistance
- Adhere to infection control, safety, and HIPAA compliance standards
- Utilizes Epic appropriately and efficiently and serves as content and workflow expert for resolving issues and proposing solutions in Epic.
- Performs other job duties as required by manager/supervisor.
Minimum Requirements
- High school diploma or equivalent
- Ability to follow directions and work under supervision in a clinical environment
- Ability to lift, transfer, and assist participants with mobility (may require lifting up to 50 lbs with assistance)
- Strong interpersonal and communication skills
- Current BLS and First Aid required
Preferred Qualifications
- Minimum one year of experience working with a frail and/or older adult population
- Minimum one year experience in adult day health, skilled nursing, or outpatient rehabilitation settings
LANGUAGE:
- Must be able to fluently speak, read and write English.
- Fluency in Chinese (Cantonese), Mandarin and/or Vietnamese is preferred.
- Fluency in other languages is an asset.
STATUS:
- This is an FLSA Non-exempt position.
- This is not an OSHA high-risk position.
