The Quality Improvement Team at Cobb Hospital has an exciting opportunity for a Patient Safety Manager!

Job Summary:


Under the leadership of the Associate Vice President or Director of Quality, the Patient Safety Manager coordinates overall high reliability education and sustainment efforts to entrench a culture of patient safety among WellStar hospital staff and faculty. This role develops and delivers education at all levels, consistent with the WellStar Safety First program, and leads the hospital's peer safety and intervention program. This role actively participates in the safety event classification process, including weekly classification activities, facilitates the root cause analysis process, ongoing common cause analyses and supports development of annual patient safety related reports and plans provided to department and organizational leadership.

Core Responsibilities and Essential Functions:


Patient Safety Program Operations
Manages comprehensive patient safety program.
Collaborates with Facility Leadership to implement and evaluate the Safety First Program including, but not limited to High Reliability Leadership Methods (Daily Safety Huddles, Leader Rounding to Influence, Fair and Just Culture Accountability, Safety Absolutes) and High Reliability (Error Prevention) Techniques
Collaborates with Facility Leadership to oversee and implement robust, highly effective safety event management processes according to WellStar Event Reporting policy (EC-60-01), WellStars standard work and best practice methodology including but not limited to: identification; reporting; reliable classification; timely investigation; cause analysis (apparent cause analysis and root cause analysis); development of corrective action plans with measures for effectiveness; and tracking of cause analyses and associated action plans.
Promotes a culture of safety by rounding on inpatient care units and ambulatory areas to perform coaching and mentoring on patient safety issues.
Develops both measurement and improvement strategies for assigned project areas. Guides the work load of assigned project areas ensuring complete and accurate patient safety and quality improvement measures and regulatory compliance requirements. Creates and implements tools for tracking and trending impact of implemented changes.
Promotes a culture of safety by coordinating the annual Culture of Safety Survey including assistance with development and tracking of action plans, as appropriate.
Supports hospital participation in the Patient Safety Organization (PSO).
Education and Program Leadership
Adheres to the general facility and system standards to promote a cooperative work environment by utilizing communication skills, interpersonal relationships, and team building.
Collaborates with System and Facility Leadership to deliver, implement and sustain the training and education program for the Safety First Program including High Reliability Leadership Methods and High Reliability (Error Prevention) Techniques
Coordinates the Safety First peer coaching and intervention program across the hospital by implementing content in collaboration with leadership to support and sustain patient safety and high reliability objectives. Serves as a subject matter expert in high reliability organizing theory and practical application.
Provides guidance and coordinates continuing education opportunities for staff to ensure professional development in patient safety and quality improvement. Participates in professional self-development, education programs, and community projects; maintains membership in professional organizations.
Manages and provides guidance for the deployment of unit level patient safety programs
Regulatory and Investigatory Adherence
Maintains current working knowledge of applicable laws, regulations, statues, and best practice literature that affect the Patient Safety Program, including Georgia Medical Board, Board of Nursing, The Joint Commission (TJC), AHRQs National Quality Forum-Serious Reportable Events (NQF-SRE) and HIPAA and ensures that services are being delivered in compliance with federal and state safety standards
Serves as consultant, subject matter expert, and facilitator, as appropriate, regarding patient safety initiatives including, but not limited to sentinel event alerts, National Patient Safety Goals (NPSG), system goals. Ensures appropriate parties are involved with timely follow-up and reliable documentation
Committee and Cabinet Oversight Participation
Collects, analyzes, recommends, and presents patient safety data and analysis of trends to hospital committees as appropriate. Facilitates process improvement that aligns with high reliability organizing theory and support the reduction of errors and other factors that contribute to adverse patient outcomes.
Provides input into the design of an effective patient safety plan through participation with hospital leadership and as an active part of the Patient Safety Committee. Serves as a liaison, ensuring coordination and consistency between the facility plan and WellStar Health Systems patient safety plan. Reviews and analyzes annually.
Financial/Fiscal Responsibility
Demonstrates leadership in the areas of human resources, financial management, planning, directing, continuous quality improvement, institutional support and representation, and customer service.
Performs other duties as assigned
Complies with all Wellstar Health System policies, standards of work, and code of conduct.

Required Minimum Education:

  • Bachelors Nursing or Bachelors Health Science

  • Masters Business Administration/Management or Masters Health Administration is preferred.

Additional License(s) and Certification(s):

  • Certified Professional Healthcare Quality Upon Hire is preferred.

Required Minimum Experience:

  • Minimum 2 years experience in healthcare, nursing or clinical practice.

  • Minimum 2 years of experience in patient safety, risk prevention, quality or education roles.

  • Experience in management of safety principles is preferred.

Required Minimum Skills:

  • Proven ability to analyze, interpret, and display data; strong attention to detail; meticulous accuracy and thoroughness

  • Superior organizational and interpersonal skills with ability to work independently as well as with a team

  • Demonstrated excellence in interpersonal and written communication skills required

  • High degree of professionalism, discretion, and confidentiality

  • Strong computer skills with Microsoft Office

  • Proficiency in problem solving and negotiating teams to reach consensus

  • Ability to build relationships and foster communication among stakeholders in clinical and non-clinical settings.
