Overview

The Wound Care APC provides comprehensive care for patients with acute and chronic wounds, including pressure ulcers, diabetic foot ulcers, venous ulcers, and surgical wounds. This role involves independent management of complex wound cases, working collaboratively within a multidisciplinary team to optimize healing, prevent complications, and ensure continuity of care across various healthcare settings. 

Responsibilities

Patient Assessment & Diagnosis: Conduct thorough wound assessments, evaluate patient history, perform physical exams, and order/interpret diagnostic tests to develop accurate diagnoses.

Treatment Planning & Implementation: Create and execute individualized, evidence-based treatment plans, including advanced wound care technologies and therapies.

Wound Care Procedures: Perform hands-on procedures such as sharp, enzymatic, and autolytic debridement, apply advanced dressings, and manage Negative Pressure Wound Therapy (NPWT).

Medication & Equipment Management: Prescribe appropriate medications and durable medical equipment (DME) to support wound healing.

Patient & Caregiver Education: Provide comprehensive instruction on wound care techniques, skin integrity, prevention strategies, and lifestyle modifications.

Collaboration & Communication: Work closely with physicians, nurses, therapists, dietitians, and home health agencies to coordinate holistic care and update referring providers.

Documentation: Accurately document all assessments, interventions, progress notes, measurements, and photographs in the Electronic Health Record (EHR).

Quality Improvement: Participate in quality initiatives, help identify quality measures, and contribute data for tracking program outcomes.

Hyperbaric Oxygen (HBO) Therapy: May participate in the care of patients undergoing HBO therapy, attend relevant meetings, and provide patient support. 

Competencies Skills

Strong clinical judgment, critical thinking, and problem-solving abilities.

Excellent communication, interpersonal, and collaboration skills.

Proficiency with EHR systems (e.g., EPIC).

Ability to adapt to evolving technology and a fast-paced environment. 

Credentials

Licensure: Current, unrestricted state licensure as a Nurse Practitioner or Physician Associate.

Certification: Board certification in wound care or relevant experience is highly desirable.

Education

Master’s degree or higher in Nursing (NP) or completion of an accredited Physician Associate (PA) program.
