Job Description
1. Maintains and actively promotes effective communication with residents/patients/clients and family members / personal representatives.
2. Responsible for maintaining a positive image of the entity in the community, keeping in alignment with our mission, vision, and values.
3. Perform resident cares according to the Individual Service Plan and as Delegated by the Registered Nurse
4. Promptly notify the on-duty Nurse of any change in resident status.
5. Provide services consistent with the resident service plan.
6. Participate in required initial and ongoing training programs and complete 15 hours of in-service training per year.
7. Use proper body mechanics in the performance of all tasks.
8. Only perform tasks in which training has been provided.
9. Always practice standard precautions.
10. Document services in an accurate and timely manner.
11. Cooperate with scheduling to accomplish adequate coverage for resident services.
12. Maintain a calm, composed attitude with compassion and empathy, while maintaining appropriate boundaries.
13. Work cooperatively with all disciplines and treat residents, families and other staff with dignity and respect.
14. Set up appointments/ transportation for families.
15. Manage call ins.
16. Audits as assigned by either Executive Director or RN.
17. Performs other similar duties, as needed.
Additional $1.00 per hour premium
QUALIFICATIONS
1. Commitment to quality programs and services for the residents/patients/clients.
2. Excellent communication and human relation skills.
3. Ability to relate well to the residents/patients/clients, family members / personal representatives.
4. Ability to maintain and protect the confidentiality of information.
5. Ability to exercise independent judgment and make sound decisions and exhibit flexibility when needed.
EXPERIENCE AND TRAINING:
1. C.N.A preferred, not required
2. Completed documented proof of CBRF Training as outlined in HFS-83
