Job Description
This is a unique position designed to support individuals served in southeastern Minnesota. The Telecommuter Case Manager will be based out of their home office, with travel to see clients in their communities. As a Waiver Case Manager, you will have the opportunity to foster trusting connections with the individuals you serve. You will play a vital role in developing comprehensive care plans, coordinating services, and connecting individuals with community resources to increase their independence and overall well-being. Through creative problem-solving and coordination with a diverse team of other health and social service professionals, you will be at the center of ensuring your clients have every opportunity to lead self-directed fulfilling lives.
