
Hospice RN Weekend On-Call Nurse - Austin and Surrounding Area
Job Description
Overview:
The Hospice RN On-Call Nurse is responsible for providing skilled nursing care and support to hospice patients during non-business hours. This role includes responding to patient and family needs, coordinating care with interdisciplinary team members, and ensuring timely interventions. The RN On Call must be able to work independently, manage urgent situations, and provide compassionate care aligned with hospice philosophy.
Essential Functions:
- Respond to patient and family calls during on-call hours.
- Provide skilled nursing care and symptom management.
- Assess patient condition and communicate with physicians and team members.
- Coordinate emergency visits and interventions as needed.
- Document all patient interactions and care provided in the EMR system.
- Support patients and families with emotional and educational needs.
- Ensure continuity of care and follow-up with the primary hospice team.
- Participate in the on-call rotation schedule as assigned.
Job Qualifications:
Education:
- Graduate of an accredited school of nursing.
- Current RN license in the state of practice.
- Hospice and Palliative Care certification is preferred.
Experience:
- Minimum of 2 years of nursing experience, with at least 1 year in a hospice or palliative care setting.
- Experience in crisis management and independent decision-making is required.
Skills:
- Strong clinical assessment and critical thinking skills.

- Proficiency in EMR systems and documentation.
