Field LPN – Home Health Visits

Activa Home Health, an AccordCare Company, is seeking compassionate and experienced Licensed Practical Nurses (LPNs) to provide skilled home health visits throughout Palm Beach County, and surrounding areas.

In this flexible per diem role, you’ll deliver patient-centered care in the home setting, including wound care, medication management, and other skilled nursing services while working alongside a supportive clinical team.
