Social Worker | Social Services | Full-Time

Responsible for consultation and provision of social services in the hospital, skilled nursing facility, emergency department, Family Medicine Clinics, all areas of campus, as needed. Provides orientation and education to the Nursing staff and other facility departments that provide direct care to patients/residents/tenants. Provides for social, emotional, and psychosocial needs of patients/residents/tenants and community; promotes continuing growth and development to the individual’s full potential. Maintains performance improvement activities within the department.
