Job Description

Sixty million Medicare seniors live with chronic disease. The care system sees most of them twice a year. Cadence is building the infrastructure to support them every day.

Cadence is a clinical AI company that delivers continuous, proactive care for older adults with chronic conditions like hypertension, heart failure, and diabetes. We pair patients with a dedicated clinical team, integrate deeply into health system EMRs and workflows, and use our Clinical Intelligence platform to monitor vitals, surface risk early, optimize medications, and close care gaps between visits. The result: patients engage with care 100x more than before Cadence, clinicians focus on judgment instead of administrative work, and Medicare saves $2M a week.

We operate as a full clinical care delivery organization, not a software vendor. Our clinicians work alongside health system partners, extending the reach of local primary care providers into patients' homes. We're now applying AI agents across these workflows – from alert review and medication titration to lifestyle coaching and care coordination – with clinicians always in control of clinical decisions.

The Role

We're hiring an Implementation Director to own the end-to-end launch of newly signed health system partners onto the Cadence platform. You'll manage every phase from contract signature through go-live, coordinating across internal teams - including solutions and integrations, clinical operations, provider relations, and partnerships - and cross-functional stakeholders at the partner organization. This role is accountable for the success of the implementation: the quality of the launch, the speed of patient volume growth, and the confidence of every operational and clinical leader who needs to trust that Cadence delivers.

What You'll Do

  • Own the full implementation lifecycle from contract signature through go-live, including project plan management, scope control, schedule, and budget accountability across each partner engagement.
  • Identify and engage key clinical, operational, and technical stakeholders at partner organizations, including those not involved in the sales process, to ensure alignment before go-live.
  • Drive stakeholder readiness by developing and maintaining a stakeholder engagement matrix that reaches down to the pilot-market level, ensuring operational and clinical leaders are prepared and aligned.
  • Lead routine and ad-hoc operational meetings with partner teams, developing and delivering content that moves decisions forward and resolves blockers before they affect timelines.
  • Maintain a live risk and issue log, escalate to health system leadership and Cadence counterparts in time for decisions to land, and track resolution through close.
  • Coordinate closely with the solutions and integrations team on EHR integration status - monitoring progress, joining relevant status calls, and surfacing risks that could affect the overall implementation timeline.
  • Scale patient volume growth post-launch by executing the go-live plan and partnering with provider relations and clinical operations to drive early adoption across the partner market.

What You Need

  • 5+ years of implementation, program management, or account management experience in or with large healthcare organizations.
  • Demonstrated experience managing complex, multi-stakeholder enterprise implementations from contract through go-live.
  • Familiarity with health system EMR environments and the role of EHR integration in clinical technology deployments.
  • Track record of managing implementation risk - maintaining issue logs, escalating to executive audiences, and driving decisions to close.
  • Experience developing and delivering content to operational and clinical leadership audiences at health systems.
  • Fluency with AI productivity tools (such as Gemini, Notion AI, or comparable platforms) and comfort applying them to structured workflows, reporting, and process documentation.

Compensation

Our job titles may span more than one career level. The base salary for this role typically ranges between $130,000 - $190,000, depending on experience, skills, seniority, and business needs. In addition to base salary, this role is eligible for equity as part of the total compensation package. Actual compensation may vary by location.
