Community Outreach Supervisor

Reporting to the Health and Wellness Manager, the Community Outreach Supervisor leads SEEDReach, a Mobile Addiction and Mental Health Outreach Team. They provide guidance and support to a multidisciplinary group with clinical insight, system knowledge, and compassion. Operating from a trauma-informed, client-centered, holistic perspective, the role addresses the needs of individuals experiencing chronic and episodic homelessness, mental illness, addiction, and other stability challenges. This includes ensuring basic needs and referrals for individuals sleeping rough through street-based interventions.
