Spiritual Care Counselors at DHHC are integral members of a multi-disciplinary health care team that are responsible for the provision of spiritual care services to patients and families/caregivers of the Agency, either directly or through coordination of care with other spiritual counselors.
The Spiritual Care Counselor will:
- Assure spiritual assessment of patients and families in the hospice program and appropriate services are rendered in a timely manner to meet regulatory guidelines.
- Provide direct spiritual care to patients and families/caregivers.
- Serve as liaison to community chaplains and spiritual counselors.
- Document direct services and ongoing communication with community resources.
- Provide funeral or memorial services for patients as requested.
- Provide bereavement support to families and caregivers for up to a year after the death of the patient.
- Perform hospice informationals for patients and families
Qualifications
- Graduate of Theological Studies from accredited university required.
- Experience with database software or electronic medical record software required.
- At least 1 unit of Clinical Pastoral Education required.
- Completion of Clinical Pastoral Education residency preferred.
- Two years of experience in health-related field preferred.
- Hospice experience preferred.
