What You’ll Do
- Conduct comprehensive evaluations and develop individualized treatment plans including OASIS paperwork.
- Deliver high-quality, evidence-based physical therapy services
- Supervise PTAs as appropriate
- Collaborate with an interdisciplinary team focused on exceptional outcomes
- Build strong relationships
- Contribute to a positive, team-driven therapy culture
- Maintain accurate and compliant documentation
What We’re Looking For
- Licensed Physical Therapist (PT) in Wisconsin
- Passion for providing home care services
- Strong clinical reasoning and treatment planning skills
- Excellent communication and collaboration abilities
- Energy, initiative, and a desire to grow professionally
- Commitment to integrity, compassion, and excellence

- Graduate of an accredited school with a minimum of a bachelor’s degree in Physical Therapy.
- Certification or license as required by the State of Wisconsin or applicable state to practice as a Physical Therapist.
- One year practicing as a Physical Therapist
