Community Health Worker

The CHW will provide support and coordination of services to older adults who participate in the LSS Healthy Transitions and Friendly Helper programs. The position is responsible for supporting older adults’ transition from hospital to home by increasing the participant’s access to resources, providing education and support that promotes stabilization and reduction of hospitalizations. The CHW will provide direct companionship care in an assigned geographic area. The responsibilities include outreach, friendly visits, light household chores, personal errands, and transportation. This position is also responsible for documenting direct care visits of members in service, assuring compliance with program regulations, and managing relationships with multiple stakeholders in the community.
