McKinley Park Care Center | Sacramento, CA

Pay: $63-$69/hour DOE

Position Summary

The Physical Therapist evaluates and treats residents, develops individualized treatment plans, collaborates with physicians and interdisciplinary team members, communicates with families, and maintains accurate clinical documentation. This role also supports staff education and contributes to quality improvement initiatives throughout the facility.

Essential Responsibilities

Clinical Care

  • Evaluate residents to determine physical therapy needs, functional limitations, and rehabilitation potential.
  • Develop, implement, and update individualized treatment plans in accordance with physician orders and facility policies.
  • Provide evidence-based therapeutic interventions to improve mobility, strength, balance, endurance, and functional independence.
  • Educate residents and families regarding treatment goals, home exercise programs, safety techniques, and fall prevention strategies.
  • Recommend appropriate durable medical equipment, including walkers, canes, splints, grab bars, elevated toilet seats, and other assistive devices.

Collaboration & Communication

  • Communicate regularly with physicians, nursing staff, residents, families, and interdisciplinary team members regarding progress and discharge planning.
  • Participate in care conferences, rehabilitation meetings, and utilization review meetings as needed.
  • Assist nursing staff with training Restorative Aides.
  • Provide educational in-services and training programs for facility staff.

Leadership & Supervision

  • Supervise Physical Therapist Assistants (PTAs), therapy aides, and students in accordance with state practice regulations.
  • Ensure therapy services are delivered safely, effectively, and in compliance with regulatory standards.

Documentation & Compliance

  • Complete timely, accurate, and compliant documentation in accordance with facility policies and payer requirements.
  • Maintain resident confidentiality and HIPAA compliance.
  • Follow infection control, safety, and equipment maintenance protocols.
  • Participate in quality assurance and performance improvement initiatives.

Qualifications

Licensure & Certifications

  • Current Physical Therapist license in the State of California.
  • License must remain active and in good standing.
  • Must meet all continuing education requirements.
  • CPR certification if required by state licensure or facility policy.
