Position: Occupational Therapist
Location: Homecare, Thompson, CT
Hours: Flexible, 32-40 hours per week
Schedule: Days, 8:00 am - 4:30 pm

Occupational Therapist Job Summary:
The Occupational Therapist (OT) provides skilled occupational therapy services to patients in their homes in accordance with physician orders, agency policies, professional standards of practice, and applicable federal and state regulations. The OT evaluates patient functional status and develops individualized treatment plans designed to maximize independence, safety, participation in activities of daily living (ADLs) and instrumental activities of daily living (IADLs), caregiver competence, and overall quality of life.
The Occupational Therapist collaborates with physicians, nursing staff, rehabilitation professionals, social workers, home health aides, caregivers, and other members of the interdisciplinary team to achieve patient-centered goals, reduce avoidable hospitalizations, and support patients in remaining safely in their homes.
Occupational Therapist Job Responsibilities:
- Performs comprehensive occupational therapy evaluations in the patient's home environment.
- Develops and implements individualized treatment plans based on physician orders, patient goals, and clinical assessment findings.
- Provides skilled occupational therapy interventions to improve functional independence, safety, and quality of life.
- Educates patients, caregivers, and family members regarding treatment plans, home exercise programs, adaptive equipment, and safety strategies.
- Collaborates with physicians and interdisciplinary team members to coordinate patient care and achieve desired outcomes.
- Maintains complete, accurate, and timely clinical documentation in accordance with agency and regulatory requirements.
- Evaluates patient progress and revises treatment goals and interventions as appropriate.
Occupational Therapist Education and Experience Requirements
Required
- Graduate of an accredited Occupational Therapy program with a Bachelor's, Master's, or Doctoral degree in Occupational Therapy.
- Current Connecticut Occupational Therapist license.
- Current BLS/CPR certification.
- Valid Connecticut driver's license.
- Reliable transportation and proof of automobile insurance.
- Ability to travel throughout the agency service area.
Preferred
- One (1) year of occupational therapy experience.
- Home health experience.
- Experience with electronic medical records and point-of-care documentation.
- Experience working with geriatric and medically complex patient populations.
Occupational Therapist Knowledge Skills and Abilities:
- Knowledge of occupational therapy principles, practices, and evidence-based interventions.
- Knowledge of home health regulations, Medicare Conditions of Participation, and Joint Commission standards.
- Ability to assess patient functional status and develop individualized treatment plans.
- Strong clinical reasoning, problem-solving, and decision-making skills.
- Ability to communicate effectively with patients, families, physicians, referral sources, and interdisciplinary team members.
- Ability to work independently and manage a flexible caseload.
- Ability to use electronic medical records and point-of-care technology.
- Ability to drive throughout the service area in varying weather conditions.
- Ability to maintain patient confidentiality and professional boundaries.
